Our social prescribing service is delivered in partnership with Community Together CIC. 

Social prescribing is a method of linking people to non-medical support within the community, with the aim of improving wellbeing. The service is free of charge and available to anyone over the age of 18. 

How could Social Prescribing benefit me? 

Your social prescriber could link you to various support to help you, including: 

🔹Feel less lonely and isolated

🔹Feel more confident and less stressed

🔹Take control of your own health and wellbeing

🔹Support you to stop smoking, or cut down on alcohol consumption

🔹Obtain support with housing difficulties or debt issues etc. 

🔹Link up with local community activities and support groups

How can I access the Social Prescribing service? 

You can speak with your GP, Healthcare professional or the practice team who will be more than happy to refer you, or you can call the Community Together CIC on 01827 59646.
